Southern Cross is a leading community healthcare provider that has been established since 1984 and is available to provide care and services throughout Australia. Our aim is to provide the highest quality care and services that assist people to remain living in their own home and to improve their quality of life.

 

Southern Cross provides a comprehensive and diverse range of services.  We are directly funded for various care and service programs from both State and Federal Government Departments and Agencies and private organisations.

 

In some instances there is no cost to the Service User for receiving Southern Cross services.

 

We deliver services to a broad cross section of the community including the catastrophically injured, the ventilator dependent, the disabled, frail aged, war veterans, young children, palliative care and post-discharge care.  Southern Cross provides short and long-term care at all levels from domestic assistance to highly skilled clinical care situations.

 

Southern Cross provides 24 hour/7 day service with support and advice available at all times through our free call National Contact Centre (Phone: 1800 989 000). The National Contact Centre provides a single point of contact and a central administration for all services.

 

Southern Cross has Care Assessors (all of whom are Registered Nurses) and Service Personnel who live and work within the local communities where the services are delivered.  A Care Assessor is nominated to be responsible for each care program and for each Service User.  The Care Assessors are responsible on a routine basis for:

*  Assessing the needs of new Service Users

*  Preparing initial and ongoing Care and Service Delivery Plans (with Service Users input)

*  Developing outcome focused Pathways

*  Identifying Outcomes

*  Attending Risk and Occupational Health and Safety Assessments

*  Monitoring the standard of care/service provided to Service Users including progress against  identified desired outcomes

*  Liaising with all relevant individuals/organisations associated with the needs of individual Service Users

*   Regular review/supervision of Service Personnel

*   Facilitating education opportunities for Service Personnel, Carers and Service Users

 

Southern Cross has successfully achieved ISO 9001:2008 Certification, Home and Community Care (HACC) Validation - Integrated Monitoring Framework (IMF), Australian Department of Health and Ageing Quality Reporting Systems, and Queensland Disability Service Standards - QDSS:2004. 

 

Furthermore, these Quality Management Systems are tangible proof of our high standards and expertise and is a confirmation to our Clients and Service Users of our professional standards, quality care, and levels of experience.
