  • Commonwealth Home Support Program (CHSP)
  • Home Care Packages (HCP)

This includes ensuring Support Workers have the skills and experience required to deliver your service and the right personality to enrich your life. You will be involved in the selection process of your Support Worker team.

Southern Cross has locally based Regional Community Coordinators (Registered Nurses) who will work with you to establish an individualised care plan and to provide ongoing support supervision to ensure the quality of the service.

The Regional Community Coordinators and the Support Workers are locally based people who live and work in the communities where your services are provided.

Activities and outings

Support Workers will assist you to be a part of the community, socialise and complete practical tasks like shopping, going on outings, paying bills and going to appointments. They can also take you to social and community activities, help you achieve goals such as playing a sport, taking you out to get your hair done, helping you shop for gifts for loved ones and visiting friends and family.

Housework

Support Workers provide a variety of domestic services to enable you to continue to live comfortably in your own home.

The Support Worker can assist with:

  • Changing bed line
  • Cleaning bathrooms
  • Cleaning kitchen benches
  • Dusting
  • Laundry
  • Vacuuming, moppping and sweeping inside and outside
    In home respite

    Support Workers will provide short term reflief for primary care givers giving them time to rest, travel or spend time with other family and friends.

    Meals and shopping

    Support Workers will assist you with daily meal preparation, cooking, cleaning and grocery shopping.

    Some of the tasks the Support Workers can assist with include:

    • Accompanying you at the shops
    • Assisting you with meal preparation
    • Cleaning up the kitchen
    • Grocery shopping
    • Providing transport to and from the shops
      Nursing

      Experienced Support Workers and/or Registered Nurses provide monitoring and support in your own home.

      The Support Workers assist you in:

      • Administering medication
      • Bowel and bladder care
      • Dressing and cleaning wounds
      • Monitoring your wellbeing
      Personal care

      Support Workers will assist you with everyday personal care tasks to help you get on with your day.

      This may include:

      • Assisting with eating and drinking
      • Dressing and grooming
      • Helping you to get in and out of bed and move around the house
      • Providing exercise therapy (as prescribed)
      • Showering or bathing
