AGED CARE SUPPORT SERVICES

Southern Cross is an Approved Aged Care Provider with the Australian Government Department of Health and Aged Care.

Southern Cross’ priority is to assist you in achieving your goals to remain living at home. We help you to achieve this through a Consumer Directed Care approach. This means that you have control over the care and services you receive including participation in the selection of Support Workers.

Cost-effective: our focus is on keeping costs down so that more of your funding can be used for the care, support and services that matter most to you. This will result in having additional funds for more services and or purchases.

We provide various quality services in your home, including:

  • Personal care
  • Domestic assistance
  • Social support – e.g. appointments, outings and shopping
  • In-home respite care

Southern Cross offers a range of personalised services in your home and local community through the following Government Programs:

  • Commonwealth Home Support Program (CHSP)
  • Home Care Packages (HCP)
  • Support At Home Program (SAH)

 

    DISABILITY SERVICES AND SUPPORT

    We provide individually tailored services to meet your needs. Your services are provided in your own home and local community with people of all ages.

    These services will help you to remain living in your own home and include:

    • Personal care (activities of daily living)
    • Complex clinical support
    • Household tasks (domestic assistance)
    • Social and community participation activities
    • Assistance in shared living arrangements (SIL)

    Southern Cross works within the Guidelines and Standards set by State and Federal Government Funded Programs therefore we are an Approved Government Provider for:

    • The National Disability Insurance Scheme (NDIS)
    • National Insurance Injury Scheme (NIIS)
    • icare NSW Schemes (Lifetime Care, Workers and Sporting Injuries Compensation, Dust Diseases, Self-Insurance)
